Definitions

HbA1c in-office testing
HbA1c testing via finger-stick provides a 3-month average of the amount of glucose attached to hemoglobin. The test is used to identify patients at risk for diabetes and prediabetes.

Policy Statement

Procedure is a non-covered dental service.

Procedure is considered medical. Submit on medical claim form with applicable medical Current Procedural Terminology (CPT®) code and diagnosis code.
